Diagnostic Methods

Color Doppler ultrasound should be the first step of diagnosis in suspicious patients.

Both the pelvic region and the patient’s legs should be examined in detail for veins with color ultrasound.

After color ultrasound, detailed examination and visualization of the veins with MR venography, CT venography, or conventional venography may be necessary for some patients.

If, as a result of the above examinations, some structural and flow-related criteria about the vessels are found, PCS may be present.

Treatment of PCS

There are a number of medications that can be used in the treatment. Several medications to restore hormonal balance can slow and sometimes stop the progression of the disease. In addition, various painkillers can be used to relieve pelvic pain.

Surgical treatment was a treatment method used mainly in ancient times. Ligation of problematic widened pelvic vessels with open surgery is almost never used today because it is a serious procedure with a high likelihood of complications.

With today’s technological advances and capabilities, closure of problematic pelvic veins using a catheter is one of the most used treatment modalities. These nonsurgical procedures, performed through the femoral vein or jugular vein, widened, structurally compromised veins in the pelvic area are detected. These problematic vessels are first treated with a vasoconstrictor and then special plugs are inserted into them. The technical success of this procedure is 99%, and the recurrence rate is less than 5%.

The procedure can be performed at any time of the year. The timing of the procedure has nothing to do with the menstrual period. Fertility and menstrual pattern do not change after the procedure.

What are the Causes of PCS?

Reasons for the development of PCS include clots and occlusions in the veins of the abdomen and genital area, obesity, which can lead to increased intra-abdominal pressure, pregnancy, constant weightlifting, and various intra-abdominal tumors and vascular pressure from other causes.

However, previous intra-abdominal surgery may also be the cause of PCS.

Complaints of Patients with PCS

PCS patients usually have abdominal and groin pain that cannot be explained by other causes, is not affected by menstruation, and may be aggravated by sexual intercourse.

The pain is accompanied by a distension in the groin and lower abdomen. In women with this condition, menstrual pain may be more severe than expected and normal.

Some patients are found to have varicose veins in the genital area, groin, inner thighs, and from the groin to the buttocks.

Most patients have varicose veins that appeared during pregnancy.

Frequent urination, distension in the anus area, blood in the urine are other complaints that may occur.

What is Pelvic Congestion Syndrome?

Pelvic pain is now one of the most common reasons for women to consult “obstetricians and gynecologists.”

Chronic pelvic pain, which occurs with a frequency of about 20% in women in society, has many causes, such as natural pain resulting from menstruation, ovarian cysts, urinary tract infections and psychiatric problems.

Pelvic congestion syndrome (PCS) is one of the most important causes of pelvic pain. However, PCS is often overlooked because it is a little-known condition and relatively difficult to diagnose.

It is a condition with symptoms such as abdominal pain, occult blood in the urine, and varicose veins in the genital area and groin, the cause of which cannot be explained by other pathologies in women.
